DOCUMENTATION CHECKLIST

The following Financial Documents are required for both the borrower and co-borrower (if applicable).

1. Mortgage Statement or Coupon (for each loan)
2. Completed Hardship Letter
3. Two most recent pay stubs for borrower and co -borrower
4. Last 2 year’s W2’s for borrower and co-borrower
5. Last 2 months’ of bank statements for checking accounts of borrower and coborrower.

FOR THE SELF EMPLOYED

1. Federal Income Tax Returns for borrower and co-borrower
   1. Copies of the original signed and dated tax returns, including all schedules for the 2 most recent tax years
2. Profit and Loss Statement for the last 3 months
3. Mortgage Statement or Coupon (for each loan)
4. Completed Hardship Letter
5. Last 3 months’ of bank statements for personal and business checking accounts of borrower and co-borrower (cover sheets only: showing the summation of deposits and debits)
